1. Confirm whether Kheir Clinic is listed as your Primary Care Provider (PCP).

  • If Kheir Clinic is already listed: You’re all set, continue to the next step to schedule.
  • If a different provider is listed: Contact our Patient Resources Department (PRD) first; they can help you update it. This matters, since your visit’s coverage depends on Kheir Clinic being listed as your PCP.
  • If your insurance plan does not require a PCP (common with PPO plans or Original Medicare): You can skip this step and continue to scheduling.

2. Schedule your clinic appointment.

3. Complete your New Patient Intake Form ahead of time.

Forms must be printed and submitted physically to front desk staff; text submission is not available to new patients.

"I'm Already a Kheir Patient and Want to Make an Appointment"

1.  Confirm Kheir Clinic is still listed as your Primary Care Provider (PCP).
Your PCP can change if your insurance renews or updates, even if you didn’t request a change.

2. Schedule your appointment.

Why do I need to confirm my PCP is Kheir Clinic?

If you have Medi-Cal managed care or certain other insurance plans, your coverage is tied to your assigned Primary Care Provider. Confirming Kheir Clinic is listed as your PCP helps ensure your visit is covered without delay.

Your assigned PCP can change if your insurance plan changes or renews, even if you haven't switched providers yourself. Contact our Patient Resources Department (PRD), and they'll help you get it corrected before your visit.

Yes. We can connect you with a provider who speaks your language directly, based on availability. If a provider isn't available, we offer interpretation services in multiple languages.
